Behavioral health & substance use

Note: Behavioral health case managers are NOT AN EMERGENCY RESOURCE. If you are in crisis or in need of immediate support, dial 911.

What is behavioral health case management?

Behavioral health case management is a free service that helps you navigate the complex system of behavioral health care and connect to available resources and services that best meet your needs when you experience barriers to access or have difficulty managing your condition.

A behavioral health case manager can help you:

  • understand your individual health care plan including how to maximize your benefits
  • coordinate care with providers, clinics, and programs to facilitate treatment for your mental health or substance use condition
  • connect to community-based services and resources for recovery, including those to meet needs for shelter, food, transportation, and legal concerns
  • enhance wellness through education, goal setting, and self-care initiatives
  • develop a support network for recovery through peer support
  • define a safety plan for crisis management.

Note: this is a non-clinical service. Our behavioral health case managers will provide supportive services and have extensive background in behavioral health, but they are not licensed therapists.

A behavioral case manager DOES NOT provide:

  • Mental health therapy
  • Crisis services
  • Help for the sole purpose of finding a therapist, psychiatrist, or other behavioral health provider
